   Registration Information
  • .For first-time patients, please bring your National Health Insurance (NHI) card and a valid ID (Household Registration, Passport, or ARC) to the registration counter.
  • .To reduce your waiting time, we recommend using the online appointment system whenever possible.

   Consultation Guidelines
  • .Please bring your ID and NHI IC card when visiting the clinic.
  • .Patients are seen in order of check-in. Those with appointments should report to the counter first and will be seen according to their appointment number.
  • .Advance registration ensures your medical record reaches the consulting room earlier, helping reduce waiting time.
  • .According to NHI regulations effective July 15, 2005, the partial co-payment is NT$80.

   Outpatient Fee Policy
  • .Patients without NHI cards must temporarily pay in cash and present the receipt and NHI card within 7 business days (excluding holidays) for a refund at the counter. Refunds requested after the end of the month must be processed through the NHI Bureau as claims will already have been filed.
  • .No additional registration fee is charged for the 2nd or 3rd refill under a continuous prescription.
  • .If examinations are rescheduled to another day, no extra registration fee will be charged for the follow-up visit.
  • .Non-NHI covered services and certificate fees must be paid according to hospital regulations.

  • .Over-the-counter or non-prescription medications

   Application for Medical Documents
  • Application for Medical Record Copies / Authorization Letter for Issuance of Medical Certificates
  • .Outpatients may verbally request their physician to issue a medical certificate. Once completed, please bring it to the billing counter for stamping and payment.
  • .For birth or stillbirth certificates, please bring both parents’ IDs or household registration to the 2nd-floor nursing station.
  • .Patients who gave birth in our hospital must provide copies of both parents’ IDs to process the newborn’s birth certificate.
  • .Requests for medical certificates should be submitted to the nursing station before discharge. The fee is NT$100 per copy, payable at the billing counter upon collection during follow-up. Requests made after discharge follow the procedure below:
      • ◎ If previously issued before discharge and additional copies are needed, please bring your ID to the billing counter to apply and pay.
      • ◎ If not requested during hospitalization, please bring your ID to the billing counter (processing time: approximately 3 business days).
  • .To request a death certificate, please bring the deceased’s ID and the applicant’s (next of kin) ID to the nursing station.
  • .Medical Record Copy (no registration required):
      • ◎ Location: Registration & Billing Counter (1F, ext. 1103 / 1105)
      • ◎ Required Documents: ID card or Authorization Letter with ID of authorized person.
      • ◎ Service Hours: Regular office hours.

For a single hospitalization, the maximum co-payment within 30 days is NT$28,000. For stays exceeding 30 days, there is no maximum limit.

   Ward Room Rate Differences
  • .Wards are categorized by room type (single or double occupancy). Rates are as follows:
  • .Observation/Standard NHI Bed: NT$0 per day
  • .Double Room: NT$2,000 per day
  • .Single Room: NT$2,500 – 3,000 per day
  • .VIP Single Room: NT$5,000 per day
  • .Each acute care room includes basic amenities such as TV and refrigerator. Rates are listed below:
